Abstrak This Reprint celebrates the 90th birthday of Professor Akira Endo, whose discovery of the first statin inaugurated a new era in cardiovascular prevention and therapy. Inspired by his work on cholesterol biosynthesis and fungal metabolites, this Special Issue brings together contributions reflecting the evolution of cardiovascular pharmacology, from lipid-lowering strategies to cardiometabolic and molecular interventions. The collection opens with a commemorative editorial by Caturano, framing the legacy and continuing impact of statins. Several articles address lipid management and vascular protection, including overviews of hypercholesterolemia treatments and studies of statins in settings such as critical limb ischaemia and cancer models. Additional work examines sacubitril/valsartan in hypertensive heart disease, nitric oxide pathway modulation after intrauterine hypoxia, and cardioprotective peptides within the counter-regulatory renin–angiotensin system. This Reprint also highlights the integration of cardiovascular and metabolic therapeutics. Contributions explore arterial stiffness, antihypertensive prescriptions, the effects of metformin and other antidiabetic drugs, statins in hepatocellular carcinoma, and novel glucose-lowering agents on stroke and cardiovascular outcomes. Reviews summarize recent advances in type 2 diabetes treatment and its cardiovascular implications.
